Preferred Membership Plan Contract Exclusions & Limitations
- Preferred Membership Plan is not an Insurance plan and cannot be used in conjunction with any type of insurance plan.
- Preferred Membership Plan can only be used at Nomad Dental Practices
- Membership can not be combined with any other promotional offer or discount.
- Must be patient of record and have had a complete comprehensive exam and full mouth x-rays within the last year.
- Plans must be paid in full (no payment plans and no refunds).
- Renewal is due on the anniversary date or before.
- *The Core Care Membership is only for patients with absence of periodontal disease which will be determined by the clinical evaluation of the dentist based on ADA guidelines.
- **X-rays include 4 Bitewings, 3 PA X-rays per year.
- ***The Emergency Exam includes: 1 Bitewing and 1 PA x-ray of the area in question.
- ****Due to interest fees paid by Nomad Dental Practices, dental services will be at a 5% discount if you use third party financing such as Cherry or Care Credit.
